LIFE COACH
Counseling Services for Children and Teens with Medicaid
MISSION
The mission of the Life Coach is to provide comprehensive
behavioral health and prevention services.
SERVICE
DESCRIPTION
Life Coach Intensive In-home Services brings counseling
services into the home and addresses the family's difficulties
together with their psychological or mental health treatment
needs.
Life Coach's clients are youth under the age of 21
who are at high risk of being removed from the home,
either because they are experiencing significant behavioral
or emotional difficulties or because they have been
subjected to abuse or neglect.
Life Coach's counselors, aptly named "Life Coaches"
work with clients and their families to reduce or eliminate
this risk and establish a stable, healthy home environment.
Each counselor is required to spend a minimum of 10
hours per week with each family for a maximum of six
(6) months per year.
WHO
IS ELIGIBLE?
Participants must qualify for Virginia Medicaid and
exhibit specific difficulties on a continuing or intermittent
basis.
In addition, at least one parent with whom the child
is living must be willing to participate with Intensive
In-Home Services, with the primary goal of keeping the
child with the family.
OTHER
GOALS
- Stabilization of the child's symptoms and behaviors
and strengthening of the family unit through crisis
intervention and intensive therapeutic services
- Enabling the family to manage the behaviors of the
child/youth more effectively in the future through
a knowledge/application of behavioral techniques
- Identification of appropriate community resources
to support the family both during the service period
and after termination of services.
- Linkage with other components in a system of care,
the intensive in-home services must conform to the
goals in the child's Individualized Service Plan as
agreed to by the parent(s) or caretaker(s) in a child
specific team meeting.
FEES
In Home Counseling Services - $70.hour
Youth Empowerment Services, Inc. is a Virginia Medicaid
provider under community based residential services.
